Elderly patients: 10 mg maximum. Treatment duration: minimum 6 months for first episode; 1–2 years or indefinite for recurrent depression. Onset of therapeutic effect: 2–4 weeks for anxiety; 4–6 weeks for depression. Discontinuation: taper by 5 mg every 2 weeks.
Side Effects and Tolerability
The most common side effects (>10%): nausea (usually resolves within 2 weeks), headache, insomnia or somnolence, dry mouth, increased sweating. Sexual dysfunction (delayed ejaculation, reduced libido) affects 20–30% of patients. Weight: neutral in short-term; mild weight gain possible with long-term use. Compared to paroxetine, escitalopram has significantly fewer discontinuation symptoms and drug interactions. QTc prolongation: avoid in patients with cardiac arrhythmias or on other QT-prolonging drugs.
Lexapro vs Cipralex: Is There a Difference?
Lexapro and Cipralex are both branded escitalopram manufactured by Lundbeck. The active ingredient, dose, and bioavailability are identical. Cipralex is the name used in most European markets (Germany, France, Italy, Spain, Poland); Lexapro is used in the US, Ireland, and some other markets.
